ACH MSA AWARD OF EXCELLENCE

This award is presented annually by the ACH Medical Staff Association to recognize a physician in the ACH community who has made an inspiring professional contribution to the ACH community over the course of their career

Call for Nominations!

This award is intended for an established physician in the ACH community who has made an inspiring professional contribution to the ACH community over the course of their career. Nominees must have been a member of the ACH medical staff for a minimum of 10 years.

The nominee should be a physician who…

• has had a meaningful impact in the areas of teaching, scholarship, research, leadership/organizational or administrative work, and/or clinical work

• has contributed to the ACH culture of excellence through their focus on physician wellbeing and advocacy

• has been a member of the ACH medical staff community for a minimum of 10 years (required)

Contributions in all areas is not required for a successful nomination; submissions will be evaluated on the quality and breadth of contributions.  Due to the nature of the award, longitudinal contributions will be given more weight.

Only ACH MSA members may submit a nomination. The nominee does not need to be an ACH MSA member.

A completed virtual nomination form must be submitted, including:

• A letter detailing the reason for nomination and recognition. Please provide specific examples of exemplary/exceptional/extraordinary accomplishments and contributions.  Please highlight the cumulative impact the nominee has made over their >10-year career at ACH.

• Letters of support from a minimum of two ACH MSA members (these may include the letter of nomination).

• Optional: A current curriculum vitae of the nominee, up to a maximum of 20 pages.
